Fungal infections cause substantial morbidity and mortality globally. While antifungal resistance is on the rise, the current antifungal treatment options include a limited number of therapeutic classes which have many pharmacokinetic (PK) and pharmacodynamic (PD) challenges. For example, much of the azole class interacts with a host of medications, requires therapeutic drug monitoring, or can result in severe side effects. Echinocandins have historically been given intravenously and daily which causes additional healthcare costs and prevents outpatient treatments. Amphotericin B, the main systematic polyene, is known to cause kidney toxicity, even in the newer liposomal formulations.

This webinar will host presentations of recent advances to overcome PK and PD challenges for antifungal treatment. Topics covered will include:

o Newer azoles that avoid drug-drug interactions.
o Extended dosing intervals.
o Echinocandins that can be used in outpatient settings with long half-lives.
o Antifungals with novel mechanism of actions and their improved activity against difficult to treat yeasts and molds.

Presentation 1: Overcoming PK/PD issues with azoles: A fosravuconazole case study (Wendy (Wan-Yu) Chu)
o Population PK-PD analysis of fosravuconazole and itraconazole in the first randomized clinical trial for eumycetoma, evaluating pharmacokinetics and exposure-response relationships.
o Model-informed dose optimization supporting 200 mg fosravuconazole as the preferred dose for future treatment.

Presentation 2: The distinct pharmacokinetic profile of rezafungin (Francelise Bridi Cavassin)
o Preclinical PK/PD profile: review of rezafungin’s pharmacometrics, stability, and pharmacokinetic parameters compared to daily echinocandins.
o Pivotal clinical trials: summary of core safety and efficacy endpoints from the clinical trial data.
o Real-world evaluation: observations on current real-world outcomes.

Presentation 3: Pharmacology of new antifungals (David Andes)
o Review pharmacologic features of new antifungals relative to existing therapeutic gaps that exist with the current antifungal armamentarium.
