This webinar will feature leading investigators of clinical trials presenting how evidence generation can support more effective, sustainable and accessible use of antibiotics in the context of rising antimicrobial resistance.

Presentation 1: One day versus 4-7 days of antibiotic treatment in patients with acute cholangitis after adequate biliary drainage (COBRA): a multicentre, open-label, non-inferiority randomised controlled trial (Anouk G. Overdevest)

  • A short 1-day course of antibiotic treatment was shown to be non-inferior to the guideline-recommended 4–7-day regimen in patients with acute cholangitis after achieving adequate biliary drainage and fever resolution.
  • A one day of antibiotic treatment after adequate endoscopic biliary drainage was found to be non-inferior to 4-7 days in adults with acute cholangitis, and reduces antibiotic use and adverse events.
  • Non-inferiority was maintained in the subgroup analysis for patients with a proven gram-negative bacteraemia.

Presentation 2: Dalbavancin as an Option for Treatment of S. aureus Bacteremia: the DOTS Trial (Nicholas A. Turner)

  • Dalbavancin was not superior to standard of care therapy for S. aureus bacteremia.
  • Dalbavancin was non-inferior by clinical efficacy.
  • While some infusion related complications were numerically less frequent with dalbavancin, numbers were too small to impact the overall outcome.

Presentation 3: ASTARTÉ trial: reviving temocillin as a carbapenem-spare drug (Jesús Rodríguez-Baño)

  • Temocillin is in vitro active against many cephalosporin-resistant Enterobacterales
  • Temocillin was non-inferior to carbapenems for the treatment of bacteremia due to cephalosporin-resistant Enterobacterales
  • Temocillin should be incorporated as a carbapenem-sparing agent for these infections
